You need to ask the printer (driver really) what it's physical and printable factors are. And, sometimes the driver lies. I had one big multi-function printer give different physical page sizes from the PCL6 driver and another from the PostScript driver. Geez!
For forms, especially with preprinted boxes and columns, I've decided to use 1/4" as my top and left margins even if the printers margins are smaller. I get the printers top and left margin and to use as an x and y offset. When I measure the start location of a field I set the ruler to the paper's physical left or top. Then the combination of the phyiscal margin and my offset brings the print to the exact spot on the page regardless of the printer model.
So, if the printer's left margin is .166" and I want to print a 1.500" from the left margin my calculation is ...
let x = (1.500 * twip.inch) - xoffset
I don't use any of the other twip values. Too much like pounds, schillings, and pence :)
Now that leaves the right and bottom margins. If the preprinted form is done right it's not usually a problem. Caveat, inkjet printers (which I hate) can have odd margins. One I dealt with had a 1/64" top margin and a 3/8" bottom margin. The forms designer put the invoice total boxes too close to the bottom and 1/2 the $ amounts were cut off.
