From what I can tell, CosW is getting an error trying to load a .dll. Do you see the "Filename: ..." as part of the message? That could tell us which one.

I Googled the "Logon failure..." message. You should do this. There's lots of info. Something to do with the Local Security Policies, and may depend on the version of Windows you're running.
