I couldn't make a small program that will "do the deed", so I've copied the actual program that caused the dump.

This is so old (4/5 years ago) that I didn't know about DECPASS/EXEPASS, so there are twice as many INCLUDEs as required

P.S. I've ZIPped all the INCLUDES into one file... hope that doesn't upset the wiki....
