qdir file corruption

Our QDIR files are becoming corrupt. Several times a week I have to rebuild one. We have about sixty directories and there does not seem to be a method to the corruption. Anyone have any ideas as to what would cause this?

What happens is I try to erase a file and then re-create and I get an error on the re-create. So the corruption stops the erase from seeing the directory entry but the create sees it and blows up with an error #12. Running a rebuild fixes it.
