Did you see any of the CosW screens in on the task bar? If so, were they sitting in QMONITOR? When this happens again it might be interesting to look in the QMONITOR keystroke file right away to see if the EXIT was really issued. The file is in COS and it's called MON-TXX, where TXX is the terminal id.
