The keystroke file doesn't show any time when EXIT was used in QMONITOR to close the session. Is it possible something caused the keystrokes to get out of sync with the programs running so the EXIT was sent somewhere other than QMONITOR?

I think the best way to track this down is for you to setup some automated test similar to what they are attempting but one that we can run repeatedly without affecting their files. Give us VNC access to the host and let us watch what happens.
