I looked in Printer Properties in my Windows Printers Folder (Start Printers and Faxes). If I set the page size here then all jobs sent to this printer will be set for this non-standard page size. That's kinda workable as this would now be the only Windows print job -- all the rest would go to the same printer under another name defined with :RAW.
It's workable, but extremely ugly..... It's sure to break when machines are upgraded, operating systems change, other jobs are designed to use Windows printing....
