Issues 1 and 3. John, I experienced the same things. However, I've been told, that you have to manually erase the original version. The quote from the documentation is as follows:
When using this mnemonic, the document is first created in the default folder associated with the printer. For pdf printers this is established in the "Properties" for the printer. For html and text printers it is the Windows temp folder. Then, when the printer is closed, a copy of the document will be made for each occurrence of the mnemonic. The original document will remain in its folder. Its full pathname can be retrieved by reading the # LUN following the CLOSE of the printer.

After closing, you have to do a DOS erase of the original file.

Re: Focus. I found that we only lose focus the 1st time that acrobat (or whatever) is launched. I had to add logic (like I've had to do for other things launched) which puts a "X" in the middle of the screen and forcing the user to click on it. I've been told there is no way of getting around it.

By the way, you can move where adobe creates the original .pdf document. I moved mine to a folder separate from the desktop and the my documents folder.
