Thought I'd try the dialog icon logic from DlgDemo.Ibs. Copied the USA flag icon into VS. Made sure the path matched. Did change the the name to USA.ICO. Looks the same to me. STDWIN.INC shows IDI.USA = 107. See VS screenshot.

When the icon didn't appear I fired up the debugger and did a screen shot right after Gosub COS.SetClassLong. cosFileSpec$ changed from C:\Comet\Customer\STD\STDWIN.DLL@00@ to "k@000000@". No idea if that's right or wrong.
