We found the problem. It could occur if there was a qdir entry for a file that didn't really exist. In Marc's case this occurred because he renamed the file using UltraEdit, which doesn't do a Comet RENAME. Thus the qdir was still expecting the file to be there. The file server was not handling this error properly.
