I've seen this too. Back to C2K4 and maybe earlier.
Win2000 Pro, single session, started manually, no QSTARTX. Seemed to be the first time after the PC was booted. Even if the PC had sat idle after the log in for 1/2 hour or more.
Came on after a SP or patch to Win2000.
