Also tried SW.RESTORE and SW.SHOWDEFAULT.
REM'd out the SW logic at the end of the program and the "fit to dialog" still fails the second time. So, it's always been like this and I didn't notice. Instead of being centered in the maximized screen (fit=false) the dialog is displayed in the top left corner.
My thinking is that, on the second entry to the dialog, the "fit to dialog" logic isn't recognizing that the COSW screen needs to be resized. QMONITOR or a (SCREEN=) resets something and the "fit to dialog" works properly again.
