INSERT Error Message

Customer currently running Comet2005 371, CometSecurity 6.02, Utility 07.01, and FileServer 6.06.
Ya, I know they need to upgrade, but they won't.
They were recently running a program that has been running for "years" with no changes. This program was ran three(3) days ago withour issue. Now they are receiving an error message - Error 96-"New Key Found During INSERT"
Looking at the hex position of the error, the program was trying to perform a WRITE when this message occurred.
So, what is COMET trying to tell me here?
