When you click on the image checkbox, DbMgr automatically recalculates the positions of each field in the file. The image checkbox is disabled for files with overlapping fields defined for the file. The recalculation process is more complicated in that case. So, it's possible your image indicator could have been turned off, then an overlapping field added, thus the checkbox would be disabled. Other than that, I don't know how you could have ended up in that situation.

Joe is correct about the position and contents of the image indicator in the #CFILES record. But, I caution you against manually changing it unless you have verified that the field positions are correct. The safest way to fix your problem was as you did - by regenerating #CFILES. Obviously, if you can discover how this indicator got changed I'd like to know.

As a reminder, Comet32 is just around the corner (really!!) and it will not support image files. Now would be a great time to convert those old files so you are ready.
