This is way out of my area of expertise but I'm going to take a wild guess that when doing FitToRect that scale isn't a factor. The width and height of the image are dictated by the width and height of the rectangle. If you're not doing FitToRect the scale settings are used to adjust the width and height of the image. In this case each bit map dimension is multiplied by (scale / 100), ie RenderedWidth = ImageWidth * (ScaleX / 100).

There's an example using FitToRect in WinPtr.ibs (DMW). It's setting the scale operands to TWIPS.PER.INCH regardless of the FitToRect option.
