used UE to look at the QDIR.DIR entries in hex:

the files that worked had "@8540@" in bytes 23/24

the one that failed had "@8440@"

all I can say with confidence is that the one that failed had a different QDIR.DIR entry, but if this was something like thinking that it was an IMAGE file... that would explain what happened... where the record counts were right, but the contents were essentially invisible
