I was able to reproduce a problem similar to what you describe. It didn't require a 6mb file however. I did it with less than 1 mb. I gave a simple demo program to Brian so he can investigate and hopefully see the problem himself. In addition to the empty filename, another symptom is a message in the CosPerr.log file that says something like:

"04/30/12 16:08:11 (11): PDF Xport file: c:\docume~1\bb\locals~1\temp\catemp\FIXA (T00)[20120430-1607042].pdf Error: The system cannot find the file specified"
