Just in case I've confused myself. Is "CA1" the program doing the failed RUN or is it the program that is being attempted by an, as yet, unnamed program?

CA1 is a bit tricky to track down. It can be run from CA or a group of other programs that are built to "revolve" from one to another without having to return to the menu. A nice idea for the VT3 era but not programmed very well :)
