I found that when I upgraded SBS 2000 to SBS 2003 and Win98 to XP, I had to also get new modems. The old modems would dial and "phone connect" and then fail before sending page 1. This was with other software so I never got to see an exact message. This was always true when sending to other fax modems. Usually true when sending to stand-a-lone fax machines.
This and all the other problems you've encountered have led me to search for a Fax Server solution for Comet. Not there yet :(
