It looks like there may be key tree damage. Like you, I would expect rebuilding by records should solve it. When I run keybuild I don't see a count of the records read - only records written. Are you using the CometExplorer rebuild or the old QTILITY?
This is puzzling. If you want, zip up the records portion of the file, tell me where the key is located in the record, and send it to me. I'll take a look.
